摘要
探讨术苓健脾胶囊(术苓)对2,4,6-三硝基苯磺酸(TNBS)诱导的大鼠实验性结肠炎的作用及其机制。采用2.5% TNBS灌肠制备大鼠实验性结肠炎模型。将大鼠随机分为正常组、模型组、肠炎宁(180 mg/kg)组以及术苓低剂量(40 mg/kg)、高剂量(120 mg/kg)组。造模后药物治疗7 d后处死大鼠。其间每日记录大鼠体重,观察大鼠疾病活动状态。实验结束后,取结肠组织进行HE病理学分析、检测结肠组织中髓过氧化物酶(MPO)酶活、诱导型一氧化氮合酶(iNOS)、炎性细胞因子(IL-6、IL-1β、IFN-γ、IL-10)及紧密连接蛋白(occludin、ZO-1)的表达水平,并检测血清中炎性细胞因子(IL-6、IL-1β)的含量。结果表明,与模型组相比,术苓给药显著缓解了TNBS造模引起的体重下降、疾病活动指数(DAI)升高,缓解了结肠组织的缩短、水肿以及病理学损伤,减少了炎性细胞浸润、隐窝的破坏与杯状细胞的丢失,降低了结肠组织MPO酶活性、iNOS与炎性细胞因子的水平,增加了结肠紧密连接蛋白的表达,同时降低了血清中的炎性因子的含量。结果表明,术苓通过降低炎性因子水平,增加肠道紧密连接蛋白的表达,改善TNBS诱导的大鼠实验性结肠炎。本研究为术苓的临床应用提供了理论依据。

本研究对象术苓健脾胶囊(简称术苓)是由骨碎补、黄柏、白芍、白术、一枝黄花、乌药、党参、铁苋菜、厚朴、茯苓、泽泻、防风、薏苡仁、大黄、桂枝、陈皮、槟榔、甘草等十八味中药组成的中药制剂。该方是在宋代官修方书《太平惠民和剂局方》中的“参苓白术散”的基础上加减化裁而来,具有健脾益肾、理气、化湿、清热的功效,临床上可以用于健脾补肾,治疗腹痛、腹泻等胃肠道疾病。基于“参苓白术散”的方解以及参考术苓的现行标准——国家食品药品监督管理局标准,并综合方中各味药的用量和功效,初步将骨碎补、党参、白术、茯苓、陈皮、白芍、薏苡仁、泽泻、黄柏定为方中君臣药。方中骨碎补肾强骨,用量最大是为君药,党参、白术、茯苓益气健脾渗湿同为君药,配伍陈皮、白芍助君药以健脾益气,兼能止泻;并用薏苡仁、泽泻、黄柏助白术、茯苓健脾渗湿,均为臣药。术苓散药性平和、温而不燥,是治疗脾虚湿盛泄泻的常用方。但是术苓对于IBD的治疗作用及其分子机制仍有待进一步的研究。
目前人类已经开发多种与人类IBD症状、形态和组织病理学相似的结肠炎动物模型。其中,2,4,6-三硝基苯磺酸(2,4,6-trinitrobenzenesulfonic acid,TNBS)与宿主蛋白结合后会引起机体抗原-抗体反应,导致透壁结肠炎,该模型症状、组织学改变以及T细胞免疫反应与CD更加类似,故多用于CD的研
术苓健脾胶囊(国药准字Z20140001,每粒0.27 g,金陵药业股份有限公司);肠炎宁片(国药准字Z36020518,每片0.42 g,江西康恩贝中药有限公司);TNBS(美国Sigma公司);水合氯醛(上海凌峰化学试剂有限公司);粪便隐血试剂盒(北京雷根生物技术有限公司);TMB显色液、RIPA裂解液和BCA蛋白定量试剂盒(上海碧云天生物技术有限公司);大鼠血清IL-6、IL-1β定量ELISA试剂盒(江苏雨桐生物科技有限公司);RNA样品稳定剂、Pro RNA提取试剂、Evo M-MLV反转录试剂盒和SYBR Green(湖南艾科瑞生物工程有限公司);4%多聚甲醛(国药集团药业股份有限公司);Tubulin抗体、occludin抗体与兔二抗(武汉三鹰生物技术有限公司);ZO-1抗体(北京博奥森生物技术有限公司)。
超纯水仪(中国力康生物医疗科技控股有限公司);SPARK多功能酶标仪(瑞士Tecan公司);7300Plus实时荧光定量PCR仪(美国Thermo Fisher公司);K960 PCR热循环仪(中国力康生物医疗科技控股有限公司);PCR基因扩增仪(中国杭州晶格科学仪器有限公司)。
50只SD大鼠适应性饲养1周,随机分为5组:对照组、模型组、肠炎宁(180 mg/kg)组、术苓低剂量(40 mg/kg)组、术苓高剂量(120 mg/kg)组,每组10只。大鼠造模前禁食不禁饮24 h,腹腔注射7%水合氯醛麻醉(300 mg/kg),液体石蜡润滑肛门管(直径2 mm)后,伸入距离大鼠肛门8 cm处,除对照组灌注生理盐水外,其余各组均缓缓推入5%TNBS与无水乙醇按2∶1体积比混合的造模剂(150 mg/kg)。大鼠倒置10 min后转移鼠笼内,等待其自然苏醒。造模当天根据分组灌胃不同剂量的药物治疗,治疗与造模同步,持续7 d。
每日观察动物疾病活动状态,称量体重,观察粪便性状及便血情况,粪便隐血试剂盒检测有无隐血。参照文献[
取动物结肠组织,采用4%多聚甲醛固定24 h,随后进行石蜡包埋、切片及苏木精-伊红(hematoxylin-eosin,HE)染色,使用倒置显微镜观察拍照后。参照文献[
取结肠组织称重,每10 mg组织加入生理盐水100 μL,在冰中用电动组织研磨器研磨得到结肠组织匀浆,12 000 r/min 离心10 min(4 ℃)后得结肠匀浆上清液。取结肠匀浆上清液10 μL,加入96孔板中,按照TMB显色液说明书指示,加TMB显色液50 μL,空白对照孔中不加样品,其余步骤相同。在室温下反应5 min。再加入浓硫酸终止液50 μL终止反应。使用酶标仪测450 nm处的吸收度,MPO相对酶活(U/g) = (样品孔测定吸收度 - 空白对照孔吸收度)/取样量(g)。随后进行统计学分析。
取小段结肠组织(于RNA样品稳定剂中保存),根据RNA提取试剂盒进行操作提取组织RNA,酶标仪检测RNA浓度,采用逆转录试剂盒将RNA逆转为cDNA,RT-qPCR检测结肠组织中的炎性因子IL-6、IL-1β、IL-10、IFN-γ、iNOS的RNA表达水平变化。实时荧光定量PCR仪调节参数:94 ℃ → 3 min;94 ℃ → 5 s;退火温度60 ℃ → 30 s(40个循环)。实验结果按照

取适量结肠组织,加入预冷的RIPA裂解液,在冰中用高速低温组织研磨仪充分研磨后,冰上裂解30 min,12 000 r/min,4 ℃,离心10 min,吸取上清液,按照BCA蛋白定量试剂盒的说明操作检测蛋白含量。使用12% SDS-PAGE进行蛋白质的分离,随后转移到PVDF膜上。转膜后使用5%脱脂牛奶室温封闭1 h,一抗(ZO-1,1∶1 000;occludin,1∶1 000;Tublin,1∶2 000)4 ℃孵育过夜,HRP标记的二抗室温孵育1 h,加入适量ECL曝光液曝光。使用Image J进行灰度分析。
血清于5 000 r/min,4 ℃,离心5 min后收集上清液。根据ELISA试剂盒说明书测定IL-6、IL-1β。简言之,将待测样品加入样品孔,在37 ℃培养箱中孵育20 min,洗涤液洗涤5次,每孔加入酶标抗体,37 ℃孵育30 min,洗涤液洗涤,重复5次,拍干;在样品孔中加入显色剂A、B,盖上覆膜,轻轻振荡混匀,37 ℃避光孵育10 min;加入终止液,终止反应,立即用酶标仪测定450 nm处的吸收度。根据标准品浓度对应吸收度制作标准曲线,并根据样品吸收度计算样品相应浓度。
与对照组相比,模型组大鼠体重显著下降(P < 0.05),并出现精神萎靡、毛色暗淡、倦怠懒动等症状,而术苓与肠炎宁给药治疗以后,体重变化率呈现出了上升的趋势,其中术苓高剂量组体重显著增加(P < 0.05,

血清中炎性因子的含量是反映机体炎症状态的重要指标。通过ELISA检测发现,与对照组相比,模型组血清中IL-6、IL-1β水平显著增加(P < 0.05),而经术苓与肠炎宁治疗后,二者浓度均显著下降(P < 0.05,

术苓由十八味中药组成。该复方中包含了多种抗炎药物。其中骨碎补在体内代谢的化学成分主要有黄酮类、三萜类、苯丙素类和木脂素类,具有保肾、消炎、降低血脂等作
本实验使用TNBS构建大鼠结肠炎实验模型,结果表明,模型组大鼠表现出严重的体重下降、腹泻伴随便血、死亡率增加等现象,而术苓给药治疗后大鼠以上症状得到缓解,初步证明了术苓对于TNBS诱导的大鼠结肠炎的治疗作用。结肠长度缩短以及黏膜充血性水肿是IBD的重要特

综上所述,术苓能显著改善TNBS构建的结肠炎大鼠的肠道炎症水平及肠上皮屏障功能。但是术苓的化学成分繁多,且化合物之间的协同作用对药效影响甚大,需要进一步探究其药效物质基础,阐明可能发挥作用的活性成分以进一步指导临床用药。
